Parent Waiver Form
I willingly give my permission for my child to engage in camps, classes and other events in and as part of DMR Adventures.
By my(our) signature below, I(we) hereby:

- Waive any claim or cause of action against and release from liability DMR Adventures, their
officers, employees, and agents for any liability for injuries to person or property resulting from
participation in the activities listed above;

- Agree to indemnify and hold harmless DMR Adventures, their officers, employees, and agents
for any claims, causes of action, or liability to any other person arising from participation in
the activity listed above;
​
- Consent to any medical treatment deemed advisable during participation in the activities
listed above. This includes treatment at the nearest facility in case of emergency.

- Acknowledge that we are signing below as the parent or legal guardian of the minor child
named above.

By answering yes to either of the questions above, you agree that once your child leaves the DMR Studio, DMR Adventures is no longer responsible for them. Additionally, where they are allowed to go is a matter between parents and students. DMR Adventures will NOT be responsible for keeping track of where students go when they leave the DMR Studio.
There may be times when one of the adults of DMR Adventures takes a group of students to one of the neighboring restaurants, either for a break or meal. There may also be times when filming or rehearsing occurs off-site at a neighboring location (such as a park).
